Patients with chronic illnesses who have a prognosis of two years or less can now receive palliative care at home while still receiving aggressive treatments for their conditions.
Palliative care is delivered by a team of nurse practitioners and nurses who collaborate with a patient's doctors to give an additional layer of support. The palliative care goal is to reduce hospitalization while managing symptoms of the chronic illness.

I was there for the first time today and all I know to say at this time is 👍WOW!
We see and read so much from the media concerning Senior Living and for whatever reason, it seems to always focus on the negative. After my visit today, my hat is off to ALL staff working at the Brightmoor facility in Griffin, GA.
I left there with a warm heart, 100 pounds removed from my shoulders, and with faith knowing that my mom will be taken care of! The facility was clean, it has that " at home" atmosphere when you step in the door, and most of all - A SMILING and COMPASSIONATE staff that seem to care about their residents.
Thanks again to everyone at Brightmoor for giving me peace of mind and to Ms. Cassandra for treating me like family! My family sends love, peace, and hugs to you all..... 💞🙏😘
